What do think the reaction would have been to Miriam's pregnancy and betrothal from her family and friends?

Perhaps 2-3 decades ago, if today's standards were applied to first century custom and culture, Miriam would have been sent away to deliver in a private setting. The husband would have been encouraged to wed her. Today we speak of teen-age ladies as "babys having babys." But that was then and the custom and culture were markedly different. God protecting Miriam from ostracism, execution, and divorcement by her fiance speaks much to God demonstrrating His love toward humanity in even caring about the details of redemption. He probably knew Gabrile was the Angel for the job if you will, that Miriam was the best suited to give birth to Yeshua, Yoseph was the man she would wed, and together they would exemplify Christian parenting in how they provided for Him and how they raised him to be God-fearing. Very remarkable indeed.
